This is announcement amendment to solicitation number 75H71323Q00039 is being issued to extend the due date. The due date is hereby extended from 12/27/2022, 10:00 AM, PT to 12/27/2022, COB, PT.





This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.



Solicitation no. 75H71323Q00039 has been issued as a Request For Quotation for a firm fixed price commercial items supply contract for custodial supplies located at Chemawa Indian Health Center, Western Oregon Service Unit, Salem, OR at one lump sum price for items on the price schedule (see Attachment 1-Schedule of Items). The performance period includes a one-year Base plus four (4) 1 -year option years. The required delivery date is December 31, 2027 for including option terms. The delivery location and place of acceptance is Chemawa Indian Health Center, Western Oregon Service Unit 3750 Chemawa Rd NE Salem, OR 97305, F.O.B. Destination.



The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-01, December 1, 2022. The provision at 52.211-6, Brand Name or Equal, applies to this acquisition. The provision at 52.212-1, Instructions to Offerors – Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation – Commercial Items, applies to this acquisition. The following factors shall be used to evaluate offers: price and past performance. Offerors are advised to submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications – Commercial Items, with its offer. 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)



(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.



(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.



(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years.

352.232-71 Electronic submission of payment requests



(a) Definitions. As used in this clause -



Payment request means a bill, voucher, invoice, or request for contract financing payment with associated supporting documentation. The payment request must comply with the requirements identified in FAR 32.905(b), “Content of Invoices” and the applicable Payment clause included in this contract.



(b) Except as provided in paragraph (c) of this clause, the Contractor shall submit payment requests electronically using the Department of Treasury Invoice Processing Platform (IPP) or successor system. Information regarding IPP, including IPP Customer Support contact information, is available at www.ipp.gov or any successor site.



(c) The Contractor may submit payment requests using other than IPP only when the Contracting Officer authorizes alternate procedures in writing in accordance with HHS procedures.



(d) If alternate payment procedures are authorized, the Contractor shall include a copy of the Contracting Officer's written authorization with each payment request.

QUOTATION INSTRUCTIONS:



Offerors shall provide quotations for each Contract Line Item Number (CLIN) based on the requirements described in the Schedule of Items. Submission shall be in the schedule of items format (Attachment no. 1) and completion of Attachment no. 3- SF1449. The respondent shall enter the unit price and extended totals for each item listed on attachment 1. If applicable, enter “equal item” manufacturer and manufacturer item number. The price shall include necessary supervision, management, labor, transportation, equipment, materials, shipping and any other direct incidentals costs, overheard and profit, unless the cost is identified as separate priced contract line item



Award will be made to the responsible respondent pursuant to FAR subpart 9.1. Quotes are due by December 27, 2022, 10:00 AM, PT.
